By completing this questionnaire, you are submitting your nomination for the position(s) indicated below. Please complete this form in full by typing responses in the space provided. The Nominations Committee will use your responses to determine whether you will be slated for a position on the Board of Directors election ballot.
NOTE: When responding to items requesting years served, please list years individually (e.g. 2020-2024 should be listed as 2020, 2021, 2022, 2023 and 2024). Please indicate “None” as a response to questions for which you do not have any information. Failure to complete this form as instructed may result in exclusion from the slate.

This completed questionnaire must be submitted no later than March 28, 2025.
If you are selected for the slate, your essay question responses, provided bio, philosophy of service and headshot will be posted online for members to view when voting.
No more than two positions may be considered. If you are applying for more than one position, ASHT will contact you once your form is submitted to determine your order of preference. The Nominations Committee will ultimately determine which nomination will go forward, based on your qualifications and noted preference.
In order to promote diversity within ASHT leadership, candidates for board services are encouraged to hold only one advanced leadership position at a time. Advanced leadership positions include, but are not limited to: voting board member, Annual Meeting Chair, ASHT Times Editor and committee chair (International, Awards, Diversity Equity & Inclusion, Leadership Development). Individuals seeking additional/multiple positions in leadership will be reviewed by the Nominations Committee on a case-by-case basis.

BOARD MEMBER COMMITMENT FORM
Exceptions to this rule will be considered for extenuating circumstances, such as outside obligations, sickness or other circumstances where board members find themselves unable to fully complete their terms of office.
Participation in Meetings and Conference Calls
- I will review all information and materials received in connection with board business.
- I will prepare for, attend and actively participate in
- monthly board conference calls
- one on-site board meeting (Summer)
- an on-site board meeting held in conjunction with the ASHT Annual Meeting (September/October)
- I will serve or assist on committees and be available to members, other leaders and staff.
Fundraising
I will support the goal of 100 percent board member participation in fundraising through an annual donation to the ASHT Vision Fund, supporting ASHT's strategic initiatives in education, advocacy, research and clinical standards. (The suggested minimum amount is $100 per year.)
Parent Organization Membership
I will maintain membership in my respective parent organizations (AOTA or APTA).
Ongoing Communication
I will respond in a timely, professional manner to all board-related communications, including those from ASHT staff.
Code of Conduct
I will abide by the ASHT Board Member Code of Conduct.
Time Commitment
The time commitment varies according to board position, but is typically 5-15 hours per month in the first year of service.
