NOMINATION INFORMATION
Description
This distinguished award recognizes a member who has made lifelong contributions to ASHT and the hand and upper extremity therapy community at large. Lifelong ASHT fellows are honored for their long-term Society membership participation, exemplary leadership, and documented achievements in the field. The cumulative effects of these recipients have been paramount in advancing the field of hand and upper extremity therapy through ASHT. Their talents have been diverse with integrated accomplishments.
Eligibility
Eligible lifetime fellows are individuals whom the Society deems worthy of special recognition in view of a career-long ASHT participation (e.g. volunteerism, Annual Meeting presentations, etc.), exemplary leadership, and documented achievements in the field (e.g. publications, programs, advanced degrees, awards, etc.). The nomination is open to ASHT members in good standing for a minimum of 25 years. Committee-endorsed recommendations will be represented to the board. Members of the ASHT Board of Directors and the Awards Committee are ineligible for nomination.
Prior to acceptance of the award, the Lifetime Fellowship Award winner shall agree to:
- uphold the ASHT Code of Ethics
- identify potential ASHT leaders using their own unique criteria
- represent ASHT if called upon as an advisor or ambassador
Process
The Awards Committee accepts nominees from the membership and selects the award winner through a structured process, presenting a non-monetary honor (plaque or equivalent award) at the ASHT Annual Meeting. A unanimous vote of the ASHT Board of Directors shall be required in order to bestow this status.
Frequency
Only one Lifetime Fellowship Award is presented each year.
Requirements
- Completed online nomination form. Nominator must write to each applicable category on the nomination form with inclusion of supportive data from the nominee’s CV. Data from the CV may not be repeated in different categories.
- Letter of nomination from an ASHT member in good standing for at least one year establishing why the nominee meets the criteria of the award. This letter is limited to a maximum of 250 words.
- Nominee’s current curriculum vitae.
This completed application must be submitted no later than August 20, 2025
